What telehealth means here
Telehealth is the delivery of healthcare using communications technology rather than an in-person visit. With ZilkerHealth this is primarily asynchronous: you complete a written intake, a licensed clinician reviews it on their own schedule, and you exchange secure messages. STATE WHETHER LIVE VIDEO OR PHONE VISITS ARE ALSO OFFERED, AND IN WHICH STATES THEY ARE REQUIRED
Your clinician is licensed in the state where you are located at the time of your visit. That is why we ask for your state before anything else.
Benefits
Access to a licensed clinician without travel or waiting rooms, on a schedule that does not require time off work.
Written records of your care that you can revisit, and ongoing messaging with your care team between formal reviews.
Risks and limitations
Your clinician cannot physically examine you. Some conditions cannot be safely assessed or treated without an in-person examination, and your clinician may decline to treat on that basis.
Care depends on the accuracy of what you report. Incomplete or inaccurate information can lead to an inappropriate recommendation.
Technology can fail. Delays, outages or transmission errors could interrupt care or affect its timeliness.
Although we use security measures, no electronic transmission is entirely free of risk of interception.
In rare cases a condition may require urgent in-person evaluation, and telehealth will delay that if you rely on it instead of seeking emergency care.
Alternatives
You are free to seek in-person care instead of, or in addition to, telehealth at any time. Choosing telehealth now does not limit that.
What is expected of you
Give complete and honest information about your health history, medications and symptoms, and keep it updated.
Follow the treatment plan you agree with your clinician, attend follow-ups, and report side effects promptly.
Maintain a relationship with a primary care provider where possible, and tell them about treatment you receive here.
Seek emergency care directly when it is needed. Call 911 rather than messaging your care team.
Records and privacy
Your telehealth encounters are documented in your medical record and handled under HIPAA and the Notice of Privacy Practices. LINK TO NOTICE OF PRIVACY PRACTICES
You may request a copy of your records at any time by contacting Hello@zilkerhealth.com.
Your consent
By proceeding you confirm that you have read and understood this document, that you have had the opportunity to ask questions, and that you consent to receive care by telehealth.
You may withdraw this consent at any time by contacting Hello@zilkerhealth.com. Withdrawing consent stops future telehealth care; it does not affect care already provided.
CONFIRM WITH COUNSEL: states requiring specific consent wording, separate written or verbal consent, or documentation of consent in the record before each encounter.
