To know the name and credentials of the clinician responsible for your care.
To receive clear information about your diagnosis, your treatment options, the expected benefits and the material risks, in language you can understand.
To ask questions and to receive answers before deciding on treatment.
To refuse any treatment, and to be told what declining is likely to mean for you.
To know the cost of your treatment before you are charged.
To privacy and confidentiality of your health information, as described in the Notice of Privacy Practices.
To see and obtain a copy of your medical record, and to request a correction to it.
To know that your clinician's recommendations are based on clinical judgment and are not directed by anyone with a commercial interest in what you are prescribed.
To raise a concern or complaint without it affecting your care, and to receive a response.
To seek a second opinion or in-person care at any time.
To continuity of care, including reasonable assistance transferring your records if you leave.
Your responsibilities
Give accurate and complete information about your health, your medications and your symptoms.
Follow the treatment plan you have agreed, and say so if you are not able to.
Report side effects and changes in your condition promptly.
Treat our clinicians and staff with respect.
Keep your account information current and your credentials secure.
Raising a concern
